Cabomba problem

What is wrong with this plant? I purchased several green cabomba 2 months back. They were doing great but recently, they have started disintegrating. Some stems are doing good, while others are just turning to mush.

SPECS-
24"x12"x12"

Flora- Contortion vallisneria, Echinodorus(quadricostatus?), Bacopa caroliniana. cabomba caroliniana
Fauna-6xDevario rerio
5xHasemania nana
2xHara jerdoni
1xBetta splendens(female)
Ferts-Seachem Flourish Iron
K2SO4
KNO3

They do have a phosphate deficiency.

DIY CO2.

Lighting is a 36 watt pll (I think colour temp. is 6400K)
